SS-31 (Elamipretide)
Elamipretide / Bendavia / MTP-131
SS-31 (elamipretide) is a mitochondria-targeted peptide that binds cardiolipin. In September 2025 the FDA gave it accelerated approval, as Forzinity, only to improve muscle strength in Barth syndrome (patients ≥30 kg). Its randomized trial missed its main endpoints; heart-failure and anti-aging use is unapproved.

How SS-31 (Elamipretide) is dosed in practice
Usually taken by subcutaneous (SubQ) injection. Typical range: 40 mg/day (SubQ). Half-life: ~3–4 hours. Each card gives the dose, frequency and duration from one evidence lane: Clinical (human trials), Expert (practitioner protocols) or Experimental (early or animal data). These ranges describe what has been studied or used; they are not a prescription.
Work out the units to draw with the reconstitution calculator, how long a vial lasts with the vial duration calculator, and how levels fall between doses with the half-life calculator.
Approved for Barth syndrome only. In the 12-week randomized crossover trial (TAZPOWER, 12 patients) elamipretide did not beat placebo on 6-minute walk distance or fatigue; knee-extensor strength rose during the open-label extension, and that intermediate endpoint supported FDA accelerated approval on September 19, 2025, with a confirmatory trial required. Label dose: 40 mg SubQ once daily. In heart failure with reduced ejection fraction, a single 4-hour infusion reduced left-ventricular volumes at the highest dose (2017), but a 4-week Phase 2 trial found no improvement versus placebo (2020).

Longevity community has adopted SS-31 as a mitochondrial rejuvenation tool. Users report improved exercise tolerance, reduced fatigue, and enhanced recovery. Use outside Barth syndrome is off-label and untested in trials for these goals. Limited long-term self-experimentation data.

SS-31 (Elamipretide): common questions
What is SS-31 (Elamipretide)?
SS-31 (elamipretide) is a mitochondria-targeted peptide that binds cardiolipin. In September 2025 the FDA gave it accelerated approval, as Forzinity, only to improve muscle strength in Barth syndrome (patients ≥30 kg). Its randomized trial missed its main endpoints; heart-failure and anti-aging use is unapproved.
How is SS-31 (Elamipretide) dosed?
SS-31 (Elamipretide) is usually taken by subcutaneous (SubQ) injection. Typical range in DoseCraft's library: 40 mg/day (SubQ). Clinical (human trials): 40 mg daily subcutaneous; Once daily; Ongoing daily therapy (label). Experimental (early or animal data): 20–40 mg daily SubQ; Once daily; 4–12 week courses. These ranges describe what has been studied or used; they are not a prescription.
Is SS-31 (Elamipretide) FDA-approved?
SS-31 (Elamipretide) is FDA-approved, but for a specific indication; most uses discussed for it are off-label.
What is the half-life of SS-31 (Elamipretide)?
DoseCraft's library lists a half-life of ~3–4 hours. Half-life sets how often a compound is dosed and how long it takes to clear: about five half-lives.
Who should not use SS-31 (Elamipretide)?
Listed contraindications: Serious hypersensitivity to elamipretide or its excipients; Severe renal impairment (label dose: 20 mg/day SubQ only in adults with eGFR < 30 mL/min who are not on dialysis; no recommended dose for patients on dialysis or children with renal impairment); Pregnancy or breastfeeding (no human data).
